Instructions
for Filling in Forms
- You must have Acrobat Reader
6.0 or higher installed. You cannot open forms with Acrobat Reader 5.0. Acrobat Reader is free and available from Adobe Systems Incorporated.
- Select the hand tool from the Acrobat toolbar menu.
- Position the "hand" over a form field and it will turn into an "I-beam". The "I-beam" indicates an area of editable text. Click the mouse to select the field for text entry.
- Use the "Tab" key to progress through the form in a logical read order. As you tab, the form fields will highlight to indicate your location on a form.
- Use the arrow pointer (hand with pointing finger)
to select a check box or a radio button. If the field is highlighted as a result of "tabbing", you may use the "space-bar" to select/deselect the box.
- Use your mouse
to select an area of the form that is not inside a form field before
printing your form (e.g., the blank margin area of the form). If a form field is active (contains the blinking bar or is highlighted) the contents may not print.
- If the fill-in
form is displayed within your Web browser's window be sure to use the
printer button on the Acrobat toolbar menu to print the form instead
of your Web browser's print function.
- It is recommended that you save
a copy of a form to your computer hard drive and use that copy to fill and print. Click here

- Adobe Acrobat Reader
does not allow you to save your changes. To save your changes you will
need Acrobat 6.0 or higher, or purchase our forms on CD-ROM.
